Assessment
How is subjective data documented
With quotation marks. EX: “The patient reports…” “The patient’s wife states…”
“The patient appears anxious.” (My inference)
Two types of subjective info:
Primary: provided by the patient
Secondary: information from a family member, chart, or other source
three ways assessment data is collected:
- Focused interview
- Physical Exam
- While reviewing lab and diagnostic test results
Where should a nurse start with a focused interview?
With the patient’s perception of their medical symptomology and how it has affected their functioning, relationships, and lifestyle.
NEVER shy away from asking questions important to the patient’s care!
What is percussion during physical exam?
Advanced technique where body parts are tapped with fingers to determine size and if fluid is present.
Hand washing should be done always at these times (6)
- Immediately before touching a patient
- Before performing aseptic technique/ handling invasive medical devices.
- Before moving from working on a soiled body site to a clean body site
- After contact with blood, body fluids, or contaminated surfaces.
- Before donning gloves and right after removing
- When leaving an area after touching a patient or their immediate environment
AIDET (common framework used to communicate with patients)
-Acknowledge: Patient by name and anyone else in room. Ask name pref.
-Introduce: self
-Duration: Timeline estimation of how long the process should take
-Explanation: step by step
-Thank you: Thank and ask if there is anything they need. If family, acknowledge their support
