Seclusive
to isolate; withdraw; shut down; stay apart from others
Re-directable
capable of being redirected in thought or action
Mannerism
a habitual gesture or way of speaking or behaving
Ritual
a series of actions or type of behavior regularly and invariably followed by someone
Compulsion
an irresistible urge to behave in a certain way, especially against one’s conscious wishes. Thought to neutralize or prevent discomfort or some dreaded event
Internal Preoccupation
being focused or engrossed in one’s own thoughts or internal stimuli
Responding to Internal Stimuli
reacting to one’s own thoughts or hallucinations by doing things such as looking around the room when no one is present. Carrying on a conversation alone, or behaving, or interacting as if someone or something else is present

Hypervigilant

Psychomotor retardation
a slowing-down of thought and reduction of physical movements in an individual.
Can cause a visible showing of physical and emotional reactions, including speech and affect (mood). Seen in depression
Tic
a sudden, repetitive motor movement or vocalization
Posturing
the taking on of an odd or inappropriate bodily position for an extended period of time
Speech
the expression of or the ability to express thoughts and feelings by articulate sounds. a formal address or discourse delivered to an audience
Monosyllabic
using brief, one syllable words during a conversation. (ex: yes, no, huh)
Clanging
speech marked by words grouped by their sound or rhyme and not by their meaning
Word salad
the use of words indiscriminately and haphazardly (disorganized, randomly) without logical structure or meaning
Neologisms
the creation of a new word. Inverted word has no real meaning except for the person; may condense several words into one. Blutter/blabber/stutter
Perseverating
abnormal, compulsive, and inappropriate repetitions of words or actions-even with varied stimuli. Repetition of a particular response
Pressured
speech that is loud and empathic (giving emphasis), increased in amount, accelerated, and usually difficult or impossible to interrupt. Forceful.
Incoherent
speech that is without meaningful connection; not understandable. Jumbled
Vague
not clear or precise in verbal expression of thoughts and ideas
Mood
how the person says they are feeling
Euphoric
feelings of intense excitement and happiness. A common symptom of mania in persons with bipolar disorder
Tranquil
free from disturbance; calm, peaceful, quiet, unagitated
Downhearted
discouraged in low spirits; unhappy. Depressed
Apprehensive
anxious or uneasy that something unpleasant might happen; concerned
Affect
the immediate and observable emotional expression of mood, communicated verbally and nonverbally. Could be congruent or non-congruent with mood
Full Range
a wide range of emotional expression; normal
Euthymic
normal mood. Non-depressed, reasonably positive mood
Dysphoric
chronic feelings of sadness, depression, unease, and loneliness
Blunted
a reduction in the intensity of an individual’s emotional expression. Individual’s show some emotion, although not as strong as what is normally expected. Dull affect; seem a little down
Flat
a severe reduction in an individual’s emotions. Person will not show signs of normal emotions, monotonous voice; lack of facial expression or reaction. Common in schizophrenia. Blank expression
Inappropriate
emotions expressed are not congruent with content of patient’s thoughts. Sad moment then starts laughing
Labile
unpredictable; sudden shifts in emotional state. Goes with being unpredictable. Ups and downs with mood
Restricted
having a narrower range of emotional expression than expected; muted reactivity
Apathetic
showing/feeling no interest, no enthusiasm, or no concern
Euphoric
feelings of intense excitement and happiness. A common symptom of mania
Congruent
the correspondence between the emotion a person is experiencing and the manner in which that emotion is being expressed. Matches mood
Incongruent
the lack of correspondence between the emotion a person is experiencing and the (way) manner in which that emotion is being expressed. Does not match mood
Thought Process
any of the mental procedures involved in cognitive activities which are past interpretation, like remembering, reasoning, problem solving, imagining, and making decisions
Loose associations
disconnected thoughts, verbal ideas shift from one topic to another. There is no apparent relationship between thoughts and the person is unware that topics are unconnected. Mind wanders from topic to topic
Tangential
thoughts that veer from the main idea and may never get back to it. Goes off on tangents
Circumstantial
a disturbance of associative thought and speech processes in which a person digresses into unnecessary, tedious details before communicating the central idea. Conversation drifts, but often comes back to the point
Flight of ideas
rapid, fragmented thoughts manifested in pressured speech. Forced out quickly(THOUGHT PROCESS)
Confabulation
unconsciously filling in memory gaps with imagined material. Not trying to deceive; this is what they think has happened or occurred
Ruminations
recurring mood-congruent thoughts. Deep thoughts; philosophical
Illogical
lack of sensible, clear, reasonable thought without reason or justification
Thought Blocking
sudden interruption in train of thought; unable to complete sentences; may be related to strong emotions or sipping into certain thoughts. Circumstantial “Going on a tangent.” (gets away from the point)
Poverty of Thought
reduction in the amount, quality, and content of thought. Pt. looks like they want to talk, but they just can’t get it out. Restriction of thoughts. Cannot think straight
Concrete
refers to thinking characterized by a focus on facts and details and inability to generalize or think abstractly. Very literal
Morbid
thoughts that are abnormal and unhealthy; interest in disturbing and unpleasant subjects-especially self-injury, suicide, death, and illness
Obsessions
unwanted, persistent thoughts or impulses; logic will not purge them from consciousness; experienced as intrusive and senseless
Phobias
strong, persistent, irrational fear of an object or situation; person feels a strong need to avoid
Coherent
thought processes that are clear and logical
Relevant
thoughts that are closely connected (makes sense to the topic) and appropriate to the situation
Irrelevant
lack of connection (doesn’t make sense to the topic) or relevancy of thoughts in relation to the situation.
Organized
thoughts that are orderly, efficient, and satisfactory
Logical
possessing sensible, clear, justifiable thought; competent reasoning
Goal Directed
often involves excessive planning of and excessive participation in multiple activities or multiple goals, thoughts, and behaviors that are goal driven
Delusion
Fixed false belief that CANNOT be changed by logical reasoning
Ideas of reference
remarks or actions by someone else that in no way refer to the person but that are interpreted as related to her/him; something happening by coincidence and believing it has a strong personal significance
Paranoia
extreme suspiciousness of others and of their actions or perceived intentions
Grandiosity
an exaggerated sense of one’s importance or self-worth or ability. (Ex: Fame or having a lot of money.)
Persecutory
thoughts that one is being harassed, treated wrongly, or even harmed in some way, fixed, false that others aim to obstruct, harm, or kill him/her
Thought Broadcasting
the delusion that others are telepathic (capable of hearing a person’s thoughts).
Thought insertion
the delusion that thoughts are being placed into one’s mind by an outsider
Thought withdrawal
the delusion that someone or something is removing thoughts from one’s mind
Hallucination
The occurrence of a sight, sound, touch, smell, or taste without external stimuli to the sensory organs.
Insight
The degree of awareness that one has about their own mental illness. Comprehension of the nature and the causes of his/her problems. Pt’s awareness and recognition that they have an illness. Ability to appreciate the benefit of treatment and define unusual thoughts and perceptions as sign of illness. Pertains to the pt’s judgement
