Addressograph machine
A machine used with the addressograph card for imprinting info on chart forms and requisitions

Chart forms used only when specific conditions of a patient call for their use
Allergy labels
Labels affixed to the front cover of a patients chart that indicates patients allergies
Chart binder/holder
Patients chart is kept together in a binder
Chart pack
Pre assembled packet f forms to be used on an admission of a patient to the nursing unit
Chart rack
Rack for holding patients chart when they’re not in use. Each slot holds one chart
Identification labels
Preprinted labels contsining individual patient info to identify patients records and specimens . Generally kept In a plastic sleeve at the front of the patients chart
Inpatient
A patient who has been admitted to a health care facility at least overnight for care and treatment
Kardex holder
Portable flip file containing the kardex forms of each patient
Name alert
A method of alerting staff when two or more patients with the same last name are located on the same nursing unit
Old chart
The patients record from previous admissions . These are stored in health records dept
Outpatient
Patient receiving care in a healthcare facility but not staying overnight
Process/transcribe physician orders
Written process used to communicate Doctor orders to nursing staff and other hospital department s
Scanning and archiving
When a patient is discharged their chart is sent to health records. There it’s scanned and saved on the computer for future purposes . The original chart is then shredded
