-FSO – SSGT BAXTER

-Air safety matters should be reported to maintain a high standard of air safety as well as prevent accidents occurring in the future

-Informs – not directly linked to air safety but can be upgraded to a DASOR if deemed serious enough
-DASOR – (Defence Aviation Safety Occurrence Report) – To be completed when a hazard or potential hazard occurrence has taken place (found on ASIMS – Air Safety Information management system.)

A

Just – Atmosphere of trust where people are encouraged/rewarded for providing safety information, where it is clear to everyone what is and is not acceptable behaviour and practices. It is not a Blame free culture

Reporting – Organizational climate where people readily report problems, errors and near misses.

Learning – Willingness and competence to draw the right conclusions from its safety information and to take appropriate action based upon those conclusion

Flexible – Adapt to
circumstances and demands whilst maintaining focus on system

Questioning – Being engaged and ready to ask “what if” and “why”. To reduce possibility of mistakes happening through assumptions

When disconnection, replacement, connection or disturbance to a control system has taken place. Its important to not only be overseen by a class 1 but also by a senior supervisor who wasn’t involved in the job to evaluate the system at a particular points.

A

-Clear of FOD prior to Working on A/C

-Loose article checks have been carried out

-Tool Control/checks (Calibrated, completeness and Condition) and Etching

Initial search (Panels removed)

760 Narrative Fault report – Fault within components (MAMP, CH-9.2)

765 Unsatisfactory Feature Report – TI/CIETP errors etc, (MAMP CH-10.1)

Role – Manage, are responsible of continued airworthiness of aircraft, Complete an Annual check of AC called a Military airworthiness review

-Develop & control maintenance programs

What are they?
What do they do?

A

DT( delivery team) – Prepare A/C for changes and mod states via Special TI, Urgent TI and Routing TI. Deal with management of maintenance & Engineering auths

ILOC (Integrated logistics operation station) – Deal with spares and how to make them readily available to us

FSR (Field service Rep) – Subject matter expert with in-depth knowledge on A/C system. Provides aid in maintenance practices.
