- Wash hands
- Introduce self as PA-S
- Explain exam and obtain consent
- Verbalize patient would be appropriately draped/exposed

General assessment
A
- Not age, race, gender, level of consciousness of Pt
- Notes if Pt is alert and oriented to person, place, time and situation
- Assess pain - noting guarding, grimacing, grunting, pallor, diaphoresis
- For children CRIS/FLACC, if indicated
- Observe emotional state

Vital Signs
A
- Would check height and weight and calculate BMI with chart
- Check Temperature, noting 97.2-99.9 F, 36.2-37.7 C normal
- Check Pulse noting 60-100 beats per minute normal in an adult, noting regular vs irregular and amplitude
- Check respiratory rate noting 12-20 breaths per minute normal in adults
- Check Pulse Ox noting 95% and up is normal
- Check BP noting < 140/90 is normal
