MacCaig is going to visit a female relative - mother? Wife? Sister? And is going through the hospital and goes through his thoughts of discomfort and general feelings of being in the hospital

What is significant about this opening of stanza 5?
It gives the image of the protagonist coming to a stand still. He’s reached the time where he must feel. His journey through the hospital is over.
What is the protagonist afraid of?
Feeling sadness and having to cope with the sight of his loved one I’ll and maybe dealing with the grief after her death.

We can tell that death is on his mind and he is finding the experience uncomfortable and distressing.
