4 phases of infectious hepatitis
1) Viral replication phase
2) Prodromal phase
3) Icteric phase
4) Convalescent phase
Phase wherein patient is asymptomatic but serologic and enzyme markers of hepatitis are present
Viral replication phase
Phase wherein patient presents with gastroenteritis-like symptoms
Prodromal phase
Phase wherein patient may note dark-colorer urine, pale stools, icterisia, and RUQ pain; on PE, hepatomegaly may be appreciated
Icteric phase (usually within 10 days of initial symptoms)
Symptoms, including icterus, resolve, and liver enzymes return to normal
Convalescent phase
Serum marker required for the diagnosis of acute hepatitis B infection
Anti HBcAg IgM
Hallmark of acute viral hepatitis
Striking elevation in serum transaminase activity, especially ALT (up to >100-fold)

HBsAg
Important diagnostic test for hepatitis per phase: Prodrome/acute disease
HBsAg (Anti-HBc)
