  1. Prescriber : Name, address, telephone number , qualification, signature.
    ——> if CD schedule 2/3 prescribed : the royal college of veterinary surgeons (RCVS) reg number must be included
  2. OWNER: name and address
  3. ANIMAL: identification and species, and address if diff to owners addy
  4. DATE : Rx are valid for 6 months or shorter if indicated by the prescriber . Schedule 2/3/4 and valid for 28 days
  5. MEDICATION : name, quantity , dose, administration , clear instruction, never “as directed”
  6. Warinings ; withdrawal period
  7. Where apt: statement of ‘prescribed under the vet cascade’
