Pathology of Meniere’s disease
Dilatation of endolymph spaces of membranous labyrinth
Presentation of Meniere’s disease
Attacks in clusters - last up to 12 hrs Progressive sensorineural hearing loss Vertigo Tinnitus Aural fullness
Ix of Meniere’s disease
Audiometry - low frequency SNHL which fluctuates
Mx of Meniere’s disease
Vertigo - cyclizine, betahistine
Gentamicin instillation via grommets
Saccus decompression
Presentation of viral labyrinthitis
Follows febrile illness eg. URTI
Sudden vomiting
Severe vertigo - exacerbated by head movement
Mx of viral labyrinthitis
Cyclizine
Improvement in days
Pathology of BPPV
Benign Paroxysmal Positional Vertigo
Displacement of otoliths in semicircular canals
Common after head injury
Presentation of BPPV
Sudden rotational vertigo < 30s
Provoked by head turning
Nystagmus
Causes of BPPV
Idiopathic
Head injury
Otosclerosis
Post viral
Dx of BPPV
Hallpike manoeuvre –> Upbeat torsional nystagmus
