What is venous reflux disease (VRD)?

A
  • Incompetence of venous valves especially in legs
  • May occur in superficial, deep or perforating veins of legs
  • Superficial involvement may cause varicose veins

What usually causes VRD?

A

Idiopathic

May have Hx DVT (post-thrombotic limb) or raised venous pressure

What are the symptoms of VRD?

A
  • Leg swelling
  • Ache
  • Burning
  • Bad cosmetic appearance

What are the signs of VRD?

A
Ulcers
Dilated tortuous superficial veins
Palpable sapena varix
Leg oedema
Venous skin changes (see venous ulcers)

A
  • Venous duplex USS

- Venography

How is VRD managed?

A

Conservative

  • compression hosiery
  • injection sclerotherapy

Surgical (only if deep incompetence excluded)

  • Ligation and stripping
  • Laser ablation of superficial vein accompanied by superficial avulsions
