Step #1
Obtain and review requisition slip to determine what equipment will be needed
Step #2
Wash hands and apply gloves
Step #3
Gather the following equipment:
- 1 Alcohol pad
- 2 gauze pads
- Tubes/vacutainers
- Band aid or tape
- Tourniquet
- Marking pen for labels
- Needle or Winged Infusion Set (if applicable)
- Hub and luer adapter (if applicable)
- Syringe (if applicable)
- Labels
Step #4
State the correct order of draw. State the order of draw for the tests that have been order. Tubes must be drawn in correct order.

Identify yourself to the patient: “Hi, my name is Rhys. I’m from the lab to draw your blood. Is that okay?”
-Must have consent to proceed.

Identify the patient by AT LEAST 2 methods:
- Ask patient for First and Last name, then have them spell First and Last name
- Hospital ID band
- Ask patient DOB

Apply the tourniquet (1 min.) and ask the patient to make a fist. Apply tourniquet 3-4 inches above site.

Use index finger of non-dominant hand to palpate the patient’s arm/hand to find the most accessible vein.

Clean the site with an alcohol pad, using sufficient friction, but not enough to abrade the skin. Or, use concentric circles from in to out.
Step #10
Allow the alcohol pad to dry completely before attempting venipuncture.
Step #11
Anchor the site, pulling distally, without contaminating the wound.
Step #12
Insert needle into the vein with the bevel facing UP at a 30 degree (ETS) or 10 degrees (winged infusion set) angle in one smooth motion.
-NEEDLE MUST NOT TOUCH YOUR THUMB

Gently insert the tube onto the needle using handles of hub/holder.
- DO NOT SWITCH HANDS.
- DO NOT CROSS OVER OR UNDER.
Step #14
- Release tourniquet and instruct patient to open fist.
- Remove the tube / invert 1 time
- Remove needle without putting pressure on the gauze

Apple pressure, if applicable, ask patient to hold pressure. Instruct the patient to not bend their arm.
