When should an incident be named
Any incident over 30 minutes
Anytime and initial IC is required to leave the command post what should happen

Location of fire by address or intersection size of fire in acres type of fuel speed of spread spread direction special instructions
Hazmat incident size up
Location of incident by address type of occupancy or carrier transporting hazmat type of material involved moving or static incident with or without fire
