Management of anterior uveitis?

A

topical steroid eye drops + cyclopentalate for pain relief (paralysis ciliary muscles)

Uveitis follow up appt
Rheumatology referral

Causes of anterior uveitis?

A

Inflammatory is most common (idiopathic, HLA-B27, sarcoidosis, Behcet’s)

Infectious (HSV, VSV, Lyme, TB, syphilis)

How would you investigate uveitis?

A

Ask about systemic symptoms in history e.g. spondylarthropathies)

Blood test for gene e.g. HLA-B27

If suspecting sarcoidosis -> CXR and Bloods (calcium, serum ACE)

Depending on aetiology, rheumatology (spondylarthropathy, sarcoidosis) or infectious diseases (TB, syphilis) may be involved

Symptoms of uveitis?

A

Painful red eye
Photophobia

(AC cells and flare, hypopyon, irregular pupil due to posterior synechaie)

Prognosis of anterior uveitis?

A

Symptoms should start to disappear after few days of treatment

Episode can last up to 6-8 weeks

Good prognosis if treated, rare to have long term visual complications

Side effect of repeated/long term use of steroid eye drops can include glaucoma and cataract however if uveitis left untreated the inflammation causes these complications much sooner
