abnormal renal tract/GU tract, voiding difficulty/obstruction, decr renal function, impaired host defences, organism eg Staph aureus
risk factors
female, sex, exposure to spermicide, pregnancy, menopause, decr host defence, urinary tract- obstruction, stones, catheter etc.
what is a lower UTI
urethra, bladder (cystitis), prostate
what is an upper UTI
pyelonephritis- renal pelvis
what is the main organism
E coli
what other organisms are involved
proteus mirabilis, klebsiella pneumonia, staph saprophyticus
symptoms of acute pyelonephritis
high fever, rigors, vomiting, loin pain, tenderness, oliguria
symptoms of cystitis
frequency, dysuria, haematuria, suprapubic pain
symptoms prostatitis
flu like symptoms, low backache, few urinary symptoms, swollen or tender prostate PR
signs
fever, abdo or loin tender, foul smelling urine
what are you looking for on urine dipstick
nitrites and leucocytes
what is the diagnostic growth
> 10^5 organisms/ml
what is pyuria
elevated WBC in urine
causes of sterile pyuria
treated UTI <2 weeks prior; inadequately treated UTI; appendicitis; calculi, prostatitis; bladder tumour; papillary necrosis; tubulointerstitial nephritis, polycystic kidney
