E coli
M/C organism of prostatitis
Gm-ve bacilli (enterobacteria : Ecoli, pseudomonas, proteus)
Management of prostatitis
Fluoroquinolones for: 4 weeks if acute, 6 weeks if chronic
Tx of acute uncomplicated cystitis
Nitrofurentoin dvery 12 hr for 5 days
Tx of pyelonephritis
Oral fluoroquinolones every 12 hr for 7 days
Tx of acute uncomplicated cystitis in males and non pregnant females
• Trimethoprim PO
or
• Nitrofurantoin PO
Tx of acute uncomplicated cystitis in pregnant females
Nitrofurantoin PO
or
• Cefalexin PO
or
• Trimethoprim PO (only in 2nd and 3rd trimester)
Tx of acute pyelonephritis in males or non pregnant females
Fluoroquinolones PO
Or
3rd cephalosporines IV then PO
Tx of acute pyelonephritis in pregnant females
• Ceftriaxone IV
or
• Cefotaxime IV
or
• Ampicillin IV + gentamicin IV
Tx of candida albicans UTI
fluconazole
Management of asymptomatic bacteriuria
• Do NOT treat asymptomatic bacteriuria except in cases of pregnant female
• Pregnant female with asymptomatic bacteriuria → treat as uncomplicated lower UTI (cystitis)
Management of catheter induced bacteriuria
• Treatment is generally only required if significant local symptoms or systemic upset (fever, vomiting)
• Start antibiotics and remove the catheter whenever possible
