What does the upper urinary tract consist of? Lower urinary tract?
Upper - kidneys and ureters
Lower - bladder and urethra
Where will MSK pain be if there is a urogenital issue?
- unilateral costovertebral tenderness
- low back, flank, pelvic, inner thigh, or leg pain
- ipsilateral shoulder pain
constitutional S&S of genitourinary disease
- fever, chill
- fatigue, malaise
- anorexia, weight loss
What are some other S&S of genitourinary disease?
- skin hypersensitivity (T10-T11)
- infertility
risk factors for genitourinary disease
- age > 60
- Hx diabetes/HTN or cardiac disease/stroke
- Hx of kidney disease - stones, UTI, obstruction
- Hx of autoimmunie
- AA, hispanic, pacific island, native american (not white)
- exposure to chemicals or drugs
- low birth weight
Where is the urogenital issue if the site of pain corresponds with dermatomes?
upper urinary tract - typically T10/T11 dermatome
What type of pain is described with upper urinary tract?
- dull/aching
- boring
may result in abdominal muscle spasm with rebound tenderness on same side
Pseudorenal pain characteristics
- acute onset associated with trauma
- affected by body position
(true renal pain is not affected by body position) - lying on side and prolonged sitting increase pain
S&S of pseudorenal pain
- bowel and bladder changes are unlikely
- negative kidney percussion test
What type of pain is described with lower urinary tract?
sharp pain felt above pubis or in lower abdomen
- pain/sensitive with going to bathroom
risk factors for lower UTI
- DM
- gout
- HTN
- obstructive urinary problems
- medical procedures requiring urinary catheterization
non-specific symtpoms of UTI
- altered mental status
- loss of appetite
- N/V/abdominal pain
cystitis vs interstitial cystitis
cystitis - inflammation + bladder infection
interstitial cystitis - inflammation only
urethritis
inflammation + urethral infection
symtpoms of interstitial cystitis
- recurring pelvic pain
- severe bladder pain/pressure
- urinary frequency
- urinary urgency
