What are causes of Urinary Tract Infections?
Most common pathogen - E. Coli
Second - Candida albicans; indwelling catheter or asymptomatic colonization.
Fungal and parasitic - Uncommon
What happens when Ureterovesical junction doesn’t work?
Patient will become incontinent.
What are some risk factors for a UTI?
- Obstructions
- Retention
- Renal Impairment
- Foreign bodies
- Anatomical factors
- Compromised immune response
- Functional disorders
- Other factors
What should you do before giving an antibiotic?
Obtain a culture prior to starting the antibiotic, and it needs to be handled quickly as well as administer fluids.
What are clinical manifestations of a UTI?
- Lower urinary tract symptoms (LUTS)
Emptying symptoms
Storage symptoms
He matures and/or cloudy appearance - Upper urinary tract symptoms
Flank pain, chills, fever, fatigue, anorexia, or asymptomatic.
Older adults; classic manifestation absent
How do we confirm a finding of a Urinary Tract Infection?
Urinalysis (UA)
What is asymptomatic bacteriuria?
Colonization of bacteria in the bladder; screen and treat with pregnancy.
How do we diagnose a Urinary Tract Infection?
- Initial: dipstick for nitrates, WBC’s and leukocyte esterase.
- Urine culture/Sensitivity
- History
- Recurring UTI’S More than 2 to 3 a yr
- Complicated UTIs
- CAUTIs or HAI UTIs
- UTI Unresponsive to empiric therapy
- Imaging ultrasound or CT scan
How do we manage a UTI?
Adequate fluids
Drug therapy - phenazopyridine and antibiotics
What do we need to teach a patient when treating a UTI?
- Drink adequate fluids to prevent kidney damage
- Urine may change in color
How do you get a patients perception of their own health?
Ask them
How should you wipe to prevent infection?
Always from the front to the back NOT back to front.
