Asymptomatic bacteriuria
Presence of significant amount of bacteria in the urine but no symptoms consistent with UTI
Only requires treatment in specific circumstances (pregnant, undergoing invasive urologic procedures)
Cardinal symptoms of an inflamed bladder
Increased urinary frequency
Pain (dysuria)
Urgency
Signs and symptoms of UTI
Dysuria Frequency and urgency Hematuria Suprapubic tenderness Chills and sweats Flank pain (costovertebral angle tenderness)
5 indications for foley catheter
End of life care if a person wants it for comfort
AKI for post renal cause
Septic shock or severe critical illness where you require accurate input and output measures
Sacral/coccyx ulcers and tissue needs to be protected
Certain types of surgery
Leukocyte esterase
Positive result means there is the presence of WBCs
Nitrite
Positive test indicates the presence of bacteria that can produce nitrate reductase
Positive does not mean UTI though
Empiric treatment for cystitis
Nitrofurantoin (5 days) or Fosfomycin (1 day)
Empiric treatment of pyelonephritis
Initial parenteral: cefriaxone or gentamycin THEN
Oral therapy: cefixime or amox-clav or cipro or TMP/SMX
In the ER/hospitalized/septic: ceftriaxone or gentamycin
Should you do a urine culture if they have
- Cloudy or malodorous urine
- Mental status change
- No
