Criteria to diagnose asymptomatic bacteriuria
- Presence of >100,000 CFU/mL of a single pathogen two successive clean catch urine cultures in a woman without symptoms
- Presence of >100,000 CFU/mL on ONE clean-catch urine culture in asymptomatic men
- Presence of 100 CFU/mL of a single isolate in a catheterized specimen in a patient without symptoms
Definition of Recurrent UTI
2 uncomplicated UTI in 6 months OR
3 or mote uncomplicated UTI in the preceding 12 months
Definition of Relapse of UTI
Recurrence of a UTI within 2 weeks of treatment completion caused by the same organism from a focus within the urinary system
Definition of Reinfection UTI
Recurrent UTI caused by a different bacterial isolate OR
previously isolated bacteria after a negative intervening culture or a period of >2 weeks between infections
Strongest behavioral risk factors for recurrent UTI in women
Frequence of sexual intercourse and use of diaphragm and spermicide
Most common pathogen in UTI
E. coli
Chlamydia infection should be suspected in the following settings:
- A new sexual partner
- A partner with urethritis
- Examination findings of cervicitis or low-grade pyuria with no bacteria seen on urinalysis
A WBC count of >____ cells/HPF in a centrifuged specimen from a symptomatic patient is considered abnormal

Treatment of choice for urethritis (adult female)
Ceftriaxone 250mg IM plus Azithromycin 1g single dose or Doxycycline 100mg BID x 7 days
First line treatment for asymptomatic bacteriuria for pregnant patients
Amoxicillin 500mg PO BID or TID for 3-7 days
Cephalexin 500mg BID-QID for 3-7 days
Treatment for HIV patients with UTI
Floroquinolones (Cipro, Levo etc)
