What kind of joint is the shoulder joint?
The shoulder joint is a ball and socket synovial joint formed by the humeral head and the scapular glenoid
What four muscles make up the rotator cuff?
- Supraspinatus
- Infraspinatus
- Teres minor
- Subscapularis
What can cause impingement syndrome?
- tendonitis subacromial bursitis
- acromioclavicular OA with inferior osteophyte
- a hooked acromion rotator cuff tear
Define impingement syndrome
This is a syndrome where the tendons of the rotator cuff (predominantly supraspinatus) are compressed in the tight subacromial space during movement producing pain
What is the classic sign of impingment syndrome?
A PAINFUL ARC
Where does pain from impingement syndrome charactersitically radiate to?
The deltoid and the upper arm
(tenderness may be felt below the lateral edge of the acriomion)
Is Hawkins-Kennedy test positive with impingement syndrome?
YES
What important differentials should be ruled out with shoulder pain?
- angina pectoris
- neck problems
- diaphragmatic irritation
- biliary colic
- heptatic abscess
- subphrenic abscess
Biceps tendonitis causes pain. Where?
Biceps tendonitis (inflammation of the tendon of long head of biceps) causes
anterior shoulder pain
with pain on resistaed biceps contraction
What causes a “Popeye deformity”?
In biceps tendonitis, the tendon can spontaneously rupture
resulting in relief of symptoms
however some are left with a bunched up biceps muscle
A patient presents with acute onset of severe shoulder pain.
This is their X-Ray. What do they have? How do you plan on managing them?
This is acute calcific tendonitis
It is characterised by calcium deposition in the supraspinatus tendon (seen on X-Ray) just proximal to the greater tuberosity
GReat relief of pain will be achieved with subacromial steroid and local anaesthetic injection
The condition is self-limiting with pain easing as the calcification resorbs
