Root: Spinal Root
Accessory n. (CN XI) [motor]
Function: motor [spinal accessory nucleus of C1-C5] innervation of the trapezius and sternocleidomastoid muscles
Brachial Plexus: (from ventral rami of spinal nerves C[4]5- T1)
Root: C5
Dorsal Scapular n. (C4-C5)
Function: postganglionic sympathetic & motor (NOT SENSORY) to levator scapulae and rhomboid muscles
Brachial Plexus: (from ventral rami of spinal nerves C[4]5- T1)
Root: C5
Long Thoracic n. (C5-C7)
Function: sensory, and postganglionic sympathetic & motor innervation to serratus anterior muscle
Brachial Plexus: (from ventral rami of spinal nerves C[4]5- T1)
Trunk: Upper Trunk (C5-C6)
Suprascapular n. (C4-C6)
Function: sensory, and postganglionic sympathetic & motor innervation of the supraspinatus and infraspinatus muscles

Trunk: Upper Trunk (C5-C6)
Subclavian n. (C5-C6)
Function: sensory, and postganglionic sympathetic & motor innervation of the subclavius muscle

Lateral Pectoral n. (C5-C7)
Function: sensory, and postganglionic sympathetic & motor innervation of the pectoralis major & minor muscles

Cords: Lateral Cord
Musculocutaneous n. (C5-C7) [two branches]
1) Muscular branches
Function: sensory, and postganglionic sympathetic & motor innervation of the biceps brachii, coracobrachialis, and brachialis muscles
2) Lateral cutaneous nerve of the forearm
Function: sensory, and postganglionic sympathetic innervation of the skin overlying the LATERAL ASPECT of the forearm

Cords: Lateral Cord
LATERAL ROOT of the Median n. (C5-C7)

Medial Cutaneous nerves of the arm and forearm (C8-T1)
Function: sensory and postganglionic sympathetic innervation of the skin overlying the MEDIAL ASPECT of the arm and forearm

Function: sensory, and postganglionic sympathetic & motor innervation of the subscapularis and teres major muscles

Axillary n. (C5-C6) [two branches]
1) Muscular branches in the arm
Function: sensory, and postganglionic sympathetic & motor innervation of the deltoid and teres minor muscles
2) Superior lateral cutaneous nerve of the arm
sensory and postganglionic sympathetic innervation of the skin overlying the SUPERIOR LATERAL aspect of the arm
Brachial Plexus: (from ventral rami of spinal nerves C[4]5- T1)
Cords: Posterior Cord
Radial n. (C5-T1) [four branches]
