pec major/minor TP location and treatment
location: belly of muscle, 3-4cm and 1-2cm medial to coracoid
treatment:
- supine
- physician on opposite side
- grab opposite arm, pronate and adduct across chest
- traction along plane toward opposite ASIS
- fine tune with traction / adduction
long head of biceps brachii muscle TP location and treatment
location: bicipital groove of humerus over long head tendon
treatment:
- supine
- physician on same side
- “patient looking at their watch”
- traction on shoulder, then move cephalad
- fine tune by external or internal rotation
upper trapezius TP location and treatment
location: anywhere along upper part of trap
treatment:
- supine
- physician at head of table
- drape patient’s arm over forearm (not wrist)
- side bend and rotate patient’s head toward TP
- fine tune by rotating head away
omohyoid TP location and treatment
location:
- on muscle at suprascapular notch
- medial to notch on scapula
- as it courses over first rib
treatment
- patient supine
- physician at head of table
- patient’s wrist over forehead, forearm-on-forearm
- passively move head toward scapula
- push hyoid bone toward side of TP
alternative: passively shrug shoulders then do above steps
levator scapulae TP location and treatment
location: upper medial portion of scapula
treatment:
- patient supine
- physician on same side
- shoulder flexed to about 30 degrees
- elbow is fully flexed and patient’s hand placed under chin
- compress along axis of humerus
supraspinatus TP location and treatment
location: supraspinous fossa just behind trap and above scapular spine
treatment:
- patient supine
- physician on same side
- flex shoulder to 90 degrees
- internally rotate shoulder
- apply traction
infraspinatous TP location and treatment
location: approx 5 cm beneatch spine of scapula, 2cm lateral to medial border
treatment:
- patient supine
- physician same side
- grab arm below elbow
- “patient looking at their watch”
- slight traction of shoulder
- forearm moved cephalad
- fine tune by internal or external rotation
rhomboid TP location and treatment
location: along medial border of scapula
treatment:
- patient SEATED
- abduct arm until
- pull arm posteriorly
alternative: bring forearm behind back
teres minor TP location and treatment
location: within belly of muscle in posterior axillary fold
treatment:
- patient lateral recumbent
- externally rotate, abduct, and slightly flex arm
- fine tune
latissimus dorsi TP location and treatment
location: within belly of muscle, from near insertion on humerus to iliac crest
treatment:
- patient supine
- extend arm to 30 degrees, internally rotate humerus, adduct slightly
- fine tune with internal rotation, extension, and gentle traction
FPR for shoulder capsule - evaluation
- patient supine
- fully flex shoulder (arms overhead)
- shortened arm means tight lat or lower trap
FPR for latissimus
- patient supine, turns head toward SD
- pronate hand, extend elbow
- little or no abduction, apply inferior traction
- other hand monitors tightness in muscle (no TP)
- re-evaluate
FPR for trap
- patient supine, turns head toward SD
- supinate hand, extend elbow, abduct arm to 15 degrees
- inferior traction
- other hand monitors tightness in muscle (no TP)
- re-evaluate
