Trapezius innervation (root, peripheral nerve)?
C3, C4 - spinal accessory nerve
Key Clinical point: most often abnormal in spinal accessory lesions caused by local surgery, with sparing of the SCM
Sternocleidomastoid (SCM) innervation (root, peripheral nerve)?
C3, C4 - spinal accessory nerve
*Most often involved in spasmodic torticollis
Rhomboids innervation (root, peripheral nerve)
C4, C5 - Dorsal scapular nerve
*dorsal scapular nerve arrises proximal to brachial plexus
Supraspinatus innervation (root, trunk, peripheral nerve)
C5, C6 - upper trunk, suprascapular nerve
Infraspinatus innervation (root, trunk, peripheral nerve)?
C5, C6 - upper trunk, suprascapular nerve
*Passes through spinoglenoid notch
Biceps brachii innervation (root, trunk, cord, peripheral nerve)?
C5, C6 - upper trunk, lateral cord, musculocutaneous nerve
*Brachialis also innervated by musculocutaneous n., however not as accessible as biceps
Deltoid innervation (root, trunk, cord, peripheral nerve)?
C5, C6 - upper trunk, posterior cord, axillary nerve
*Motor unit action potentials may have increased polyphasia in normal subjects
Teres minor innervation (root, trunk, cord, peripheral nerve)?
C5, C6 - upper trunk, posterior cord, axillary nerve
Brachioradialis innervation (root, trunk, cord, peripheral nerve)?
C5, C6 - upper trunk, posterior cord, radial nerve
*May be abnormal in lesions of radial nerve at or proximal to spiral groove
Serratus Anterior innervation (root, peripheral nerve)?
C5, C6, C7 - long thoracic nerve
Pectoralis Major innervation (root, trunk, cord, peripheral nerve)?
C5, C6, C7, C8, T1
- upper, middle, and lower trunks
- medial and lateral cords
- medial pectoral nerve (sternal) - medial cord C8, T1
- lateral pectoral nerve (clavicular) - lateral cord C5, C6, C7

C6, C7 - upper/middle trunk, lateral cord, median nerve
*May be spared in pronator syndrome
*Spared in AIN syndrome

Extensor Carpi Radialis Longus (ECRL) innervation (root, trunk, cord, peripheral nerve)?
