6
Q

Describe the pin index safety system (PISS).

A

Pin configuration on each hanger yoke assembly is different for each gas tank.
Goal is to prevent inadvertent misconnections. Note: having more than one washer may bypass the PISS system.

Q

Describe the Diameter Index System (DISS).

A

Each gas hose and connector are sized and threaded for each individual gas line.
Goal is to prevent inadvertent misconnections of gas hoses.

Q

What are the max pressure and volumes for cylinders of air, O2, N2O?

A

Air (yellow) = 625L, 1900 PSI, PIN 1,5
O2 (green) = 660L, 1900 PSI, PIN 2,5
N2O (blue) = 1590L, 745 PSI, PIN 3,5

Q

Bourbon pressure gauge on O2 cylinder reads 500 psi. If flow rate 4L/min, how long with this cylinder provide O2 to patient?

A
660L/1900PSI = x/500PSI = 174L
174L/(4L/min) = 43.5 min.

Q

Is it ever safe to use an O2 cylinder in the MRI suite?

A

only if made of non-magnetic material such as aluminum.

MRI cylinders have 2 colors: silver tank with only the top covered with either yellow (air), green (O2), blue (N2O).

Q

List 3 safety relief devices that prevent a cylinder from exploding when the ambient temperature increases.

A

gas cylinders should never be exposed to temps > 130 F (57 C).

  1. fusible plug made of Wood’s metal (melts at high temps)
  2. frangible disk that ruptures under pressure
  3. valve that opens at elevated pressures.

Q

Give 1 example of how the O2 pressure failure device (failsafe) might permit the delivery of a hypoxic mixture.

A

if there is a pipeline crossover, the pressure of 2nd gas will produce pressure to defeat the failsafe device.

13
Q

Give 4 examples of how the hypoxia prevention (proportioning device) might allow the delivery of a hypoxic mixture.

A
  1. O2 pipeline crossover
  2. leaks distal to the flowmeter valves
  3. administration of a third gas (helium)
  4. defective mechanic or pneumatic components

14
Q

What is the difference between O2 pressure failure device and the hypoxia prevention safety device?

A

O2 pressure failure device = failsafe device that shuts off and/or proportionally reduces N2O flow if O2 pressure drops < 20 psi.
Hypoxia prevention safety device is a proportioning system that prevents the user from setting a hypoxic mixture with flow control valves. Max N2O = 75%

15
Q

Describe the structure and function of flowmeters.

A

annular space is the area between the indicator flow and the side wall of the flow tube. narrow at the base and wide at the top.
It has a variabel orifice that provides constant gas pressure throughout a range of flows.
1. Laminar flow depends of viscosity (Poiseuille)
2. Turbulant flow depends on gas density (Graham).

16
Q

What is the safest flowmeter configuration on the anesthesia machine?

A

O2 flowmeter should be furthest to the right/closest to the manifold outlet.
If there is a leak in the other flowmeters, presents delivery of hypoxic mixture.

17
Q

How do you calculate FiO2 set at the flowmeter?

A

FiO2 = (air flowrate x 21) + (O2 flowrate x 100)/ (total flow rate)

18
Q

An anesthesia machine uses fresh gas coupling. How do you determine the total tidal volume that will be delivered to patient?

A

Vt total = Vt set on ventilator + FGF during inspiration

FGF during inspiration = (ml/min X I:E)/ breaths per min.

19
Q

When using a ventilator that couples fresh gas flow to tidal volume, what types of ventilator changes impact the TV delivered to patient?

A

Vt increases with: slow RR, increased I:E, fresh gas flow, and bellows height.
Vt decreases with: high RR, decreased I:E, fresh gas flow, bellows height.

20
Q

What is the vaporizer splitting ratio?

A

Modern variable bypass vaporizers split fresh gas into parts:

  1. some FGF enter vaporizer and saturated with 100% IA.
  2. The rest of gas bypasses chamber and doesn’t pick up any agent (dilutes saturated gas).
