Summary of what matters to a person, produced on a single page of A4 piece of paper
Must have:
1. What people like and admire about the individual
2. What is important to the individual
3. How to support the individual.
How are routines a tool to find out what is important for them
Getting an individual to talk through their daily routines will provide an idea of what is and what isn’t working for them. For example, if they tell you that it typically takes two hours to get out of bed and dressed, they may need further support in their morning routine.
How is finding our good days and bad days helpful for finding out information
Requires the individual to describe typical good and bad days, so that the care worker can help support what is needed to achieve a good day. When it comes to bad days, factors can be identified and avoided so that, the day is improved
How are relationship circles used as a tool to find information
Helps identify who the individual knows and how close they are. The networks helps who is closer and further away incase of an emergency and to help support the individual
How are top tips/ two minute drift used as a tool to find info
Aim is to learn what’s important to and for the individual, the tool is used for when the care worker only has a few moments with the service user.

What are personal budgets
An agreed amount of money that is used to carry out or deliver certain aspects of provision set out in an individuals support plan.
There are two types of ways a personal budget may be taken. These are direct payments and managed accounts
What are direct payments
Direct payments are a method of person budgets. The payment consists of a service giving an agreed amount of money to the individual instead of having to provide them care. This is so they can arrange their own care, and spend it appropriately. They need to keep accurate records
What are managed accounts
Managed by the local authority in line with the persons wishes. This many include paying for community care services provided
It is unlikely that a managed arrangement will offer the same level of user choice and control as a direct payment
What is co production
Co production is about collaborating together to empower individuals.
Benefits of personalisation
Individuals gain and maintain control
Individuals remain in their own home when receiving care
Inclusion in community
Improved infrastructure and guidance
Improved quality of life, self-esteem and socialisation
Positive impacts of personalisation
Direct payment for care allows more availability for support
Inclusion within communities
Remaining home makes familiarity
Access to info and guidance allows for better choices

Challenges of personalisation
- Care is limited to the prescribed budget
- Availability and access to some services may be restricted, especially if in a rural area and travel is difficult because of poor public transport
- worry about spending the budget as all the money has to be accounted for
