Boggy fundus and trickle bleeding
Massage fundus
Uterine atony
Massage fundus and make sure bladder is empty
Fundus off to R or L side and boggy
Take pt to bathroom or use a bed pan if unable to ambulate or use an in and out catheter if cannot go. Empty the bladder and massage fundus until firm
Fundus firm and blood spurting
Signs of a cervical or perineal laceration
S/S infant infection
Tachypnea
Poor feeding
Temp
Mastitis
Could be due to bad latch causing lack of emptying breasts, cracked sore nipples, engorgement, warm sore breasts
Diapraghm
Cervical cap
Contraceptive sponge
All need to be left in for a minimal of 6h after intercourse to full kill the sperm
PPH
Postpartum hemorrhage caused by uterine Atony or improperly closed laceration
Non breast feeding
Bind breasts for 72h
Use ice packs
Use chilled cabbage leaves
Avoid warmth
Do not express milk
Avoid baby rooting
Should be gone by 4-7 days
Breastfeeding
Warmth
Feed baby often, may need to wake baby to feed
Make sure baby has a good latch
Colostrum for first 48h then turns to milk
Carboprost
Give lomotil for diarrhea
Contraindicated for Asthma and HTN
Methylergonovine
Contraindicated with Pre eclampsia and HTN
Circumcision care
Petrolatum gauze for 24h
Apply generous amounts of petrolatum to each diaper for 7-10 days
Assess bleeding first hour every 15min then hourly for 4-6h
Apply sterile gauze if bleeding occurs
Document first void post procedure
