*Most common cause of UGI bleed
Characterized by a break in the mucosa that comes in contact with pepsin and HCL

Gastric Ulcers
Found at the junction of the fundus and pylorus, but most on the less curvature of the antrum, a break in the mucosal barrier that extends into the muscularis mucosae.
Agents that break the mucosal barrier:
- ASA
- ETOH
- NSAIDS
- H. pylori

Duodenal Ulcers
A chronic break in the duodenal mucosa extending through the muscularis mucosae that leaves scars
Most common type of peptic ulcer
assoc with increased acid secretion
90% of pts test positive for H. pylori infection
Duodenal Ulcers is associated with
Increased acid secretion.
Acid secretion is stimulated by protein rich meals, calcium, and vagal stimulation

Stress Ulcers
Found in critically ill patients
Stress Ulcers are caused by:
- Acid hypersecretion
- Gastric anoxia (hypoperfusion to stomach)
- Altered mucosal defense mechanism
- Decreased mucosal pH (becoming more acidic)
Those at risk for stress ulcers
- Severe trauma
- Sepsis
- Severe burns
- Cranial or central nervous system dz
- Patients with long-term ventilatory support
