Arterial insufficiency?
Almost always due to atherosclerosis.
Dt diabetes, smoking, high cholesterol BP obesity, heart disease
Two mechanisms for veins?
Calf muscle pumps blood uphill during exercise.
One way valves prevent back flow.
If these fail -> pooling -> increased venous pressure -> fibrin deposition around capillaries -> reduced oxygen and nutrients to muscle and skin -> ulcer
Diabetic?
Combination of arterial blockage and nerve damage.
Sensory neuropathy reduced awareness to pressure heat pain
Diabetic characteristics?
Similar to arterial
More located over pressure points such as heels or between toes
With pressure callus formation then breaks
Arterial characteristics?
Punched out gray base
Found on lateral side of ankles feet
Frequently painful
Intermittent claudication
Venous ulcers?
Located below the knee, inner ankle
Painless
Sayer ounces by dry itchy and reddened skin (eczema)
Thickened skin, scaliness, tiny rough bumps
What is the Braden scale?
Six criteria for risk of pressure ulcer
- sensory perception
- moisture
- activity
- mobility
- nutrition
- friction and shear
