UE Flashcards
Describe the neuritis chapmans points of the upper limb
Anterior-a gangliform contraction between the 3-4 rib near the sternal border. Association of extreme pain in upper arm and hand; worse at night
Posterior-Between 3-4 dorsal transverse processes, between transverse and spinous processes. Associated with 3rd rib depression and lock soft tissues of the foot.
The adson’s test is for what?
Test for tight scalenes. Patient extend, abduct, and externally rotate, then look to the ipsilateral side. Take a deep breath and turn head.
Wrights test is for what? How do you do it?
Test for compression under pec minor and coracoid process.
I have a question test
Costoclavicular syndrome (military posture test)
Tests compression between clavical and first rib
What would you call damage to a single nerve root in the cervical region? What would you observe?
Cervical radiculopathy: dermatome, myotome, and reflexes would be affected.
What’s a brachial plexopathy? What would this affect, and what is an example?
Damage to trunk, cord, or branches affected multiple levels. Dematome, myotome and reflexes affected.
Example is thoracic outlet syndrome.
What would you call damage to a single nerve branch? What are some examples of conditions like this?
Mononeuropathy- ulnar, radial ect.
carpal tunnel, cubital tunnel
A patient comes in c/o a loss of sensation to the right index finger and you notice some atrophy of the flexor carpi radialis and pronator teres. What neurologic condition is most likely and which nerve is involved?
1) cervical radiculopathy, radial
2) cervical radiculopathy, median
3) mononeuropathy, radial
4) mononeuropathy, median
Bonus- what dermatome would you check; nerve root?
4) mononeuropathy (one nerve branch)
median nerve innervates the index finger
C6
You have a decreased deep tendon reflex at C6 which nerve is being tested?
Radial for the brachioradialis muscle
You have a cervical radiculopathy affecting C5-6 and notice a decrease in abduction of the right arm, as evidenced by a positive arm drop test. Which muscle is most likely affected?
A) Supraspinatous
B) deltoid
C) teres minor
D) serrratus anterior
B) deltoid. Although, the supraspinatous also abducts the test looks at the first 15 degrees at which point the deltoid muscle is doing most the abduction.
The entrapment of the ulnar nerve occur in the cubital tunnel formed by the medial epicondyle of the humerous and what other structure?
A. radial head
B. ulnar head
C. olecranon of the ulna
D. radial protuberance
olecranon of the ulna
Your patient is a writer for the New Yorker and has recently been dropping things at work and at night feel like she has to shake her hand to wake it up. She’s otherwise in good health. What type of nerve injury does she most likely have?
A) cervical radiculopathy
B) cervical radiculitis
C) brachial plexopathy
D) mononeuropathy
D mononeuropathy affecting the median nerve branch
This is carpal tunnel syndrome- could treat with interosseus membrane release or Stills carpal tunnel tech
Which tendons like deep to the median nerve in the carpal tunnel? Which structure makes up the superior boarder of the tunnel?
Flexor tendons
Flexor retinaculum
When treating an anterior radial head with muscle energy which motion should the physician resist?
Resist supination (posterior resist pronation)
