Piver
Type 1 hysterectomy
“simple hysterectomy”
Type 2 hysterectomy
“modified radical hysterectomy”
medial half of the cardinal and uterosacral ligaments
Uterine artery taken at its junction with the ureter
Upper one fourth (1-2cm of vagina) also removed.
Type 3 hysterectomy
“Wertheim/Meigs-Okabayashi hysterectomy”
cardinal and uterosacral ligaments completely transected and one third to one half of the vagina is removed. Uterine artery taken at origin.
Autonomic nerves for bladder and rectal function also resected.
Type 4 hysterectomy
cardinal and uterosacral ligaments completely transected at their origin
One half of the vagina removed
Superior vesical artery sacrificed
All periureteral tissue is removed
Type 5 hysterectomy
for tumours which invade the lower urinary tract. Involves removing portions of the bladder and distal ureters.
