What is a pharmaceutical care plan?
- an integral component of the practice of comprehensive pharmaceutical care
- written document of the pharmacist’s assessment of the actual and potential drug therapy problems for an individual patient and
- a record of the goals, outcomes and monitoring associated with this assessment.

According to Hepler and Strand’s definition, what is the intent of pharmaceutical care?
To achieve definite outcomes that improve a patient’s quality of life
What kind of outcomes therefore, should PCP’s include?
ones that are realistic and measureable.
What kind of outcomes are there?
Outcomes measureable by biological and phyiological function, e.g. peak expiratory flow rate, blood pressure, blood glucose
more complex measures like patient functioning (e.g. improved mobility), patient satisifcation with care
How is patient assessment carried out?
Pharmacist performs thorough assessment of patient’s entire drug therapy needs to identify any current or potential problems
pharmacist uses systematic process to analyse relevant patient, drug and disease information to produce this assessment.

How is patient consultation carried out?
- gather necessary information
- discuss patient’s expectations, concerns on drug therapy
- identify patient’s drug-related needs
- pharmacist to elicit info about patient’s condition and other relevant medical history, perform the necessary tests like peak flow rate assessment, check prescription and other pharm records about pharmaceutical usage, contact other health pofessionals as required, check medication understanding and compliance
document info using hard copy documentation form or appropriate software

purpose of this step is for PCP to go into effect
pharmacist may provide individualised current info and education about patient’s drug therapy
give instructions for proper medication and related product use
demonstrate any techniques or devices or provide appropriate health/disease info
may be necessary to refer patient back to GP with recommendations of change of therapy
all interventions documented
how is patient monitoring and followup carried out?
monitoring and followup by pharmacist occurs at planned intervals to ensure new drug therapy problems do not develop, therapeutic goals are being met and actual patient outcomes are evaluated and documented.
What happens if goals are not being met?
The full process will be repeated
What coudl follow up and monitoring involve?
regular contact at the pharmacy by phone, or in some instances by home visit, according to individual cicumstances
What does the PCP offer?
