[Tropho] Gestational Trophoblastic Neoplasms Flashcards
Tumor that invades the myometrium and is characterized by trophoblastic hyperplasia and persistence of placental villous structure
Invasive Mole
Epithelial malignancy of trophoblastic cells formed by the abnormal proliferation of cytotrophoblasts and syncytiotrophoblasts in the absence of chorionic villi
Choriocarcinoma
Rare neoplastic proliferation of intermediate trophoblasts that invade the myometrium at the placental site after a pregnancy
Placental Site Trophoblastic Tumor
Rare trophoblastic neoplasm which is the malignant counterpart of the intermediate trophoblasts of the chorion leave
Epithelioid Trophoblastic Tumor
Risk Factor for Choriocarcinoma
- Previous H. Mole
2. Advanced Maternal Age
Most commonly seen initial presenting symptom for GTN
Abnormal Vaginal Bleeding
Symptoms of GTN
- Abnormal Vaginal Bleeding
- Enlarged Uterus
- Acute abdomen secondary to tumor perforation
- Infection from bulky necrotic tumor
Tumor Marker used for GTDs
Beta hCG
Pattern of Metastatic Spread
Hematogenous
Most Common Site of GTD Metastasis
Lungs (80%)
For the scoring system, this is used for counting the number of metastasis in the lungs
Chest X-Ray
GTN Therapy can be initiated when these criteria are met
- Clinical presentation consistent with GTN
- Persistently elevated/Rising B-hCG levels
- Presence of Intrauterine Mass
- Documentation of one or more metastatic lesions
Cornerstone of GTN Treatment
Chemotherapy
First line chemotherapeutic agent for GTN
Methotrexate
Regimen for giving Methotrexate
5 day regimen every 7-10 days
