What physiologic processes occur in trochanteric bursitis?
inflammation in the lubricating sac located between the midpoint of the trochanteric processes of the femur and the gluteus medius tendon/ iliotibial tract (deep= deeper sac)
What is the typical MOI?
exaggerated movement of the gluteus medius tendon and the tensor fascia over the outer femur. (repetitive flexing and direct posture aggravate)
- Gait disturbance causes MOST
What are some common gait problems that can cause trochanteric bursitis?
- lumbosacral spine disease
- leg length discrepancy
- sacroiliac joint disease
- knee arthritis
- ankle sprain
What are the symptoms?
- lateral hip pain over outer thigh
- difficulty walking
- pressure sensitivity
Where is the point of maximal tenderness?
1.5 inches below the superior portion of the trochanter (directly over the maximum lateral prominence)
When are radiographs recommended?
in all patients with suspected trochanteric bursitis
