Antifolate antibioitc

How does it work?
A
Oral bacteriostatic dihydrofolate reductase inhibitor. Folate is an essential cofactor in DNA synthesis so this is why it is bacteriostatic.

Indications?
A
Mainly UTIs – (E.coli, proteus mirabilis).
Acute/chronic bronchitis, shigellosis, invasive salmonella infections.

Containdications?
A
Pregnancy (teratogenic effects). Severe renal impairment. Blood disorders (e.g. anaemia, thrombocytopenia).

Side-effects?
A
Rare: bone marrow suppression, nausea, vomiting, rash, toxic epidermal necrolysis.

Possible interactions?
A
- Cyclosporins: ↑risk nephrotoxicity
- Pyrimethamine: ↑ antifolate effects.
