A sudden, unilateral, severe, brief stabbing, recurrent pain in the distribution of one or more branches of the 5th CN
What is the incidence of trigeminal neuralgia?
3-5/ 10,000
Women> men
Often are 50+
What are 3 types of trigeminal neuralgia?
Idiopathic
Classical
Secondary
What is the presentation of trigeminal neuralgia?
Unilateral
Sharp shooting pain
Paroxysms
Describe: knife, electric shock
When in the day is it more common to see trigeminal neuralgia?
Day > night
Increased activity
What is the pathophysiology of trigeminal neuralgia?
Local demyelination of trigeminal nerve
Nerve loses myelin allows jumping of electrical signals
What structure is between pons (brain) and trigeminal ganglion?
Dorsal root
What artery overlies the nerve and what significance is this?
Inferior cerebellum artery
Causes of classical, secondary and idiopathic tN?
Classical - blood vessel
Secondary - underlying pathology
Idiopathic - unknown
What are secondary causes of TN?
Multiple sclerosis
Tumour cerebellar-pontine angle
Peripheral neuropathy
Skull base deformity
What would you see on MRI of pt w/ MS and TN?
White plaques of demyelination
