Signs and symptoms

A

o Asymptomatic in 50%

o Symptomatic:

  • Green/yellow “frothy” vaginal discharge
  • Vulval itch or vaginal soreness
  • Offensive odour
  • Lower abdominal pain and dysuria
  • Dyspareunia

Investigations

A

· Investigations:

  • High vaginal swab -> direct microscopy (wet mount of vaginal fluid shows flagellated organism in the middle)
  • Endocervical swabs for other STIs
  • pH (>4.5 – only in BV and TV)
  • Whiff test
  • Culture and gram stain
  • HIV test, NAAT, VDRL

Management

A
  • 1st line = Metronidazole, 400mg BD, PO, 7 days
  • 2nd line = Metronidazole, 2g, PO STAT

Contact tracing, abstinence for 7 days, follow-up

Complications

A

Pregnancy = PTL, LBW, PPROM (preterm labor, low birth weight, premature rupture of membranes)
