Treatment Modalities Flashcards
Ischemic Heart Disease
(coronary artery disease)
abovel all mangage risk factors!!!
Prevent Recurrent Ischemic Episodes
Organic Nitrates: sulingual nitroglycerin for acute attacks and prophaylaxis, isorbide dinatrate (or mononitrate) for longer acting (beware of tolerance and dose to allow drug free interval
Beta blockers and Calcium channel blockers: beta blockers reduce myocardial oxygen demand by prevening increase in HR and contractility. Calcium channel blockers reduce myocardial oxygen demand (vendilation reduce preload/ventricle size/stress and arterial dialtion which reduces resitance) and increas oxygen supply (cornonary arterial dilation) also good for vasospasm
Prevent Acute Cardiac Events
Antiplatelet therapy (aspirin, clopidogrel which can substitute for aspirin if allergic but also given together with aspirin) l_ipid regulating therapy_, maybe ACE inhibitors
**Revascularization **
Percutaneous transluminal coronary angioplasty (PTCA) and CABG (coronary artery bypass surgery)
Acute Coronary Syndrome
Diagnosis
Symptoms
EKG
Serum Markers
Imaging: echocardiogram can be used if diagnosis of MI is uncertain. Look for abnormality of contraction in area of ischemia
