What is the pathway that we follow to treat sepsis?
Sepsis 6
What components make up the sepsis 6?
- Oxygen
- IV fluids
- IV antibiotics
- Lactate
- Urine output
- Blood cultures
How much oxygen should be given?
15 litres initially then titrated to target saturations (94-98%)
What device should be used when delivering the oxygen?
Non re-breathe mask
How much oxygen (%) does a non-rebreathe mask deliver?
60-98%
When should blood cultures be taken?
Prior to antibiotic therapy
What else can be taken to determine the source of infection?
- Urine
- Wound
- Routine bloods
When should antibiotics be given?
Preferably after blood cultures have been taken
What antibiotics will be given?
Refer to trust policy until blood cultures come back
Why should fluids be prescribed?
- To reduce organ dysfunction
- Increases organ perfusion
- Possible improved cardiac output
What type of fluids should be prescribed?
A crystalloid (usually hartmanns)
How much fluid should be prescribed?
maximum 30ml/kg (divided into boluses of approx 10ml/kg)
What is lactate?
Lactate is a stress response. It is located all over the body.
What are routine bloods?
- Full blood count (FBC)
- Urea and electrolytes (U&E’s)
3 Liver function test (LFT’s) - Clotting
What can be done to monitor urine output?
- catheter insertion
