Treatment Flashcards
Goal
50% reduction on LDL levels
Decision to treat
estimate risk using baseline levels
Recommendations: age 20 years older, not on lipid medications
Measurement of either a fasting or nonfasting level is effective
Recommendations: In adults 20 years + with nofasting trigylceride lves > 400
repeat lipid pabel in fasting state
Recommendations: LLDL < 70
measurent of diretc LDL or modified LDL os resonable to improve accuracy
Recommendations: 20yeas+ w/o personal hx ASCVD but fhx of premature ASCVD or genetic hyperlipidemia
measure fasting lipid profile
Lifestyle modifications: Diet
fruit, vegetables, fiber, low fat dairy, poultry, fish, nontropical oils is ok; limit sweets, refined carbs, red and processed meats; avoid trans fats
Lifestyle modications: Physical activity
stimulates activity if lipopreitn lopase; lower tricglerscies and VLDL, promote CV fitnees and weih loss; 150 minutes/week moder teinteisyt or 75 minues;wee of vigros areboci exerse (swimming, bikin, playing teniis)
Re-assessment
Low risk patients starting with TLC: 2-3 visits in 3 months
Recommendations for drug thx
(1) Select patient with moderately high LDL > 160 (2) Patients with very high LDL > 190. (3) High intensity statin plus ezetimibe and/or PCKS9 inhibitor for the pt at very high risk (hx multiple major ASCVD events)
