Gunshot wound to upper zone of neck without indications for surgical exploration – management? If base of neck?
Arteriographic management
Arteriography, esophagogram, esophagoscopy, bronchoscopy
Penetrating injury to neck – surgery if?
Surgical exploration if:
- Expanding hematoma
- Deteriorating vitals
- Signs of esophageal or tracheal injury (spitting up blood)
Asymptomatic patient with stab wound to upper or middle zone of neck – management?
Observation
Patient with severe blunt trauma to neck – get CT if?
CT scan in patients with neurologic deficits or patients with pain to local palpation of cervical spine
Hemisection of spinal cord – typical cause? Symptoms?
Clean-cut injury (nice)
- Paralysis and loss of proprioception ipsilaterally
- Loss of pain perception contralaterally
Anterior cord syndrome – typically seen with? Symptoms?
Burst fractures of vertebral bodies
- Loss of motor function, pain, and temperature sensation on both sides
- Preservation of vibration and positional sense
Central cord syndrome – typical patient? Typical cause? Symptoms?
Elderly
Forced hyperextension of neck (rear end collision)
- Paralysis and burning in upper extremities
- Preservation of function in lower extremities
Management of spinal cord injuries?
- MRI
