Transfusion Reactions Flashcards
1
Q
Acute reactions
A
- Acute hemolytic
- Febrile nonhemolytic
- simple allergic
- severe allergic/anapylactic
- TACO
- TRALI
- sepsis
2
Q
Delayed reactions
A
- delayed hemolytic (immune-mediated)
- alloimmunization to RBC Ag
- transfusion-associated graft-vs-host disease
- transfusion-transmitted diseases
3
Q
Acute hemolytic TR
A
- during transfusion (within minutes)
- up to 4h post-transfusion
4
Q
Febrile nonhemolytic TR
A
- during transfusion or up to 4-6 h post-transfusion (relatively common)
5
Q
Simple allergic TR
A
- During transfusion or up to few hours post (very common 1:3-1:300)
6
Q
Severe allergic/anaphylactic TR
A
- usually during transfusion (within min)
7
Q
TACO
A
- during transfusion or up to a few hours post
- commons (1:100) - higher is susceptible pt (CHF)
8
Q
TRALI
A
- during and up to 6 h post
- not as common 1:5000
9
Q
sepsis
A
- usually during or within an hr post
10
Q
Delayed hemolytic TR
A
- 3 d - 2 or more weeks post transfusion
11
Q
Alloimmunization to RBC Ag
A
- IgM response - 10d - 2wks post
- IgG response - 3 or more wks post
- relatively common
12
Q
Transfusion-associated GVHD
A
- 2-30d post (most commonly within 4-10d post)
- mortality virtually 100%
13
Q
Transfusion transmitted dx
A
- days to years post
- HIV, HCV, HBV
14
Q
Treatment/Management
A
- STOP transfusion
- confirm correct blood product given
- abx for sepsis symptoms
- diuretics for volume overload
- transfer to ICU for TRALI
