Tracheostomy tubes Flashcards
Where are trach tubes implanted?
Upper airway obstruction at or above level of vfs
Why are trach tubes inserted?
To prevent potential upper airway obstruction (edema post surgery)
-respiratory care
where are they inserted?
btween 3rd and fourth ring
What are the parts of the trach tubes?
Outer cannula
Inner cannula
Obturator
What are some types of trach tubes?
Cuffed
Cuffless
Fenestrated
Cuffed
Prevents aspiration used with ventilators and inflated for increased pressure
Cuffless
Aspiration may occur May interfere with laryngeal elevation during swallow
Assists with breathing and secretion removal
long term use
Fenestrated
Smaller
weaning and decannulation
short term (3-5 days)
what is used for voice when someone is trached?
Passy Muir
Provox
steps to laryngectomy
1 remove larynx
2 resection hyoid, pre epiglottic space, thyroid cart, cricoid and trach rings
3 hypopharyngeal constictor ms are sectioned
4. trachea is sutured to skin
how do they reduce fistula formation?
constrictor ms are usually reconstructed in the midline to support a one or two layer mucosal closure
communication post laryngectomy
electrolayrnx
esophageal speech
TEP
What does TEP require?
Respiratory support
Cognition *occusion of stoma andcleaning prosthesis
adequate vibratory pe segment
What are some complication that occurs with TEP
aspirationof food and prothesis stenosis stome and fistula infection spasm migration or fistula enlargement
