what is a thyroid storm?
A
sudden surge of large amounts of thyroid hormones which cause increase in body metabolism.

what are clinical manifestations of thyrotoxicosis?
A
- hyperthermia
- hypertension
- fever that is 104-105
- delirium and seizures
- fast dysrhythmias/chest pain/palpitations

what are lab findings for a patient with a thyroid storm?
A
T3 and T4 are elevated

what are priority interventions for a patient with a thyroid storm?
A
- maintain airway
- initiate continuous cardiac monitoring
- monitor I&Os hourly
- cool patients with a sponge bath or ice packs to help with the hyperthermia
- IV fluids are given to prevent vascular collapse due to the fluid volume deficit

what medications can be given for a thyroid storm?
A
- acetaminophen for fever
- propylthiouracil and methimazole to stop excess production of thyroid hormones
- propranolol to control HR and BP
- glucocorticoids for adrenal insufficiency or shock
- no aspirin can be given because it increases thyroxine levels

what are complications of a thyroid storm?
A
- restlesness that looks like impending sense of doom, stroke , MI
- thyroidectomy complications
- hypocalcemia
- heart failure

what are thyroidectomy complications?
A
airway obstruction, laryngeal edema, stridor, hemorrhage, and tracheal collapse

what are signs of hypocalcemia and what is the antidote?
A
- tingling of fingers, muscle twitching, Chvostek’s and Trousseaus, seizures
- give calcium gluconate as antidote
