What changes in thyroid diseases?
Body weight, appetite, voice, bowel habit, energy level, mood, concentration, heat intolerance, skin, nails and hair, heart rate/rhythm, menstrual cycle
In hyperthyroidism, how does body weight change?
Decrease despite increased appetite (although 10% will gain weight)
How does hypothyroidism change body weight?
It increases it
How does hyperthyroidism change appetite?
Increases it
How does hypothyroidism change appetite?
It will decrease it
How does hyperthyroidism affect voice?
It doesn’t
How does hypothyroidism affect voice?
It causes hoarseness
How does hyperthyroidism change bowel habit?
It increases the frequency of defaecation or causes diarrhoea
How does hypothyroidism change bowel habit?
It causes constipation
How does hyperthyroidism change your energy level?
It can increase or decrease your energy level. It causes muscle weakness.
How does hypothyroidism change your energy levels?
It decreases it. You are somnolent. You get slower movements and muscle weakness.

Anxious, iriitable, emotionally labile

Lethargy, depression

Increased sweating, fine hair and hair loss. Clubbing. Onycholysis.

Dry flaky skin, coarse thinning hair, brittle nails
How does hyperthyroidism affect heart rate/rhythm?
Tachycardia, palpitations, atrial fibrillation
How does hypothyroidism affect heart rate/rhythm?
Bradycardia
How does hyperthyroidism affect menstrual cycle?
Infrequent menses, fertility problems
How does hypothyroidism affect menstrual cycle?
Heavy, prolonged or irregular menses. Fertility problems and increased risk of miscarriage.
What drugs are especially relevant for thyroid history?
Thyroxine, lithium, amiodarone and sulphonylureas
What social history is relevant for thyroid history?
Smoking, alcohol, diet (sufficient iodine) and effect on activities of daily living
