How should a patient be positioned for a thyroid examination?
Patient sitting upright
Neck and top half of sternum exposed
What should be identified during general inspection?
Restless/ relaxed
Lethargy/ disinterest
Paraphernalia
What should be identified when examining the head and face?
Large tongue
Periorbital puffiness (puffiness around the eyes)
Dry, brittle hair
Hair loss from outer 1/3 of eyebrow (hypo)
What vital signs should be taken?
Radial pulse (rate/ rhythm to identify tachycardia or bradycardia) Offer BP
What should be identified when examining the hands?
Sweaty palms (hyper)
Carpel Tunnel syndrome
Tremor (hyper)
What should be identified when examining the nails?
Clubbing (Graves disease)
Onycholysis (hyper)
What should be identified when examining the eyes?
Exophthalmos
Lid lag
Lid retention
Diplopia
What should be identified when inspecting the neck?
Neck swellings (patient should swallow water) Scars/ surface changes Asymmetry
What should be palpated?
Lymph nodes Thyroid cartilage (patient should swallow water)
What should be percussed?
Percuss down sternum to identify retrosternal goitre
What should be auscultated?
Carotid bruits
Stridor
What should be examined peripherally?
Ankle reflexes (patient kneeling on chair) Pretibial myxoedema
What additional examinations should be offered?
Examination for Carpal Tunnel Syndrome
Muscle power
