What is a goiter and what can cause it?
enlargement of the thyroid gland, caused by hypothyroidism
Manifestations of hypothyroidism?
Drowsiness, confusion
bradycardia
anorexia, weight gain, edema
constipation
fatigue, weakness
intolerance to cold
dry, sparse coarse hair
depression
Indication of Levothyroxine? routes?
hormone replacement therapy oral, IV
adverse effects of levothyroxine?
Thyroid storm causing:
tachycardia, palpitations, chestpain, increased BP, anxiety, tremors, insomnia, weight loss, heat intolerance
Long term use of levothyroxine causes?
osteoporosis
contraindications of levothyroxine?`
clients with hypertension
interactions of levothyroxine?
decreases effectiveness of insulin,
increases excretion of digoxin,
increases effects of warfarin,
food decreases absorption of levothyroxine
Implications of levothyroxine
base vitals
monitor tachycardia and irregular pulse
client teaching of levothyroxine
report HCP if :
heart palpitations, chestpain, or symptoms of thyroid storm present
do not discontinue, lifelong use.
notify b4 switching brands
take in AM on empty stomach
monitor TSH and T4 levels periodically
