Hypothyroidism
Primary: abnormality in the thyroid gland itself
Secondary: when cause is other body system dysfunction
Example: pituitary gland is dysfunctional and does not secrete TSH
Hypothyroidism drug
levothyroxine (Synthroid)
•Synthetic T4
•Treatment of all forms of hypothyroidism
ADVERSE:
•Cardiac Dysrhythmias, tachy
•Weight loss
•Palpitations
Levothyroxine Client Care Implications
•Teach clients to report any unusual symptoms, chest pain, or heart palpitations
•Take levothyroxine 30-60 min before breakfast
Hyperthyroidism
Excessive thyroid hormone secretion
Increased activity of multiple body systems (opposite to hypothyroidism)
●CV (eg palpitations, increase HR)
●CNS stimulation (nervousness, rapid thought/speech, insomnia)
●High metabolic rate (heat intolerance, skin flushed and moist)
●Muscle weakness and atrophy
●Increased appetite but weight loss
Hyperthyroidism drugs
Radioactive iodine (131I)
•Destroys thyroid gland
Antithyroid drugs: thioamide derivatives
Methimazole
•Prevent thyroid hormone synthesis
