Thymus Flashcards

1
Q

Encapsulated and multilobulated thymic tumour, with intersecting fibrous bands. Tumour cells are spindled to ovoid with bland nuclei, finely dispersed “powdery” chromatin, inconspicuous nucleoli and absent mitoses.

A

Thymoma - Type A

Plan:
Correlate with clinical history and radiology (?myasthenia gravis)
Examine further blocks (e.g. for size, relation to margins, component of type ‘B’ morphology to indicate AB thymoma, TNM / Masaoka-Koga stage)
IPX to support Dx: AE1/AE3, p40
Next steps: Synoptic report, discuss at MDT

Q

Encapsulated and multilobulated thymic tumour, with intersecting fibrous bands. Comprised mostly of densely packed small lymphocytes, with areas of medullary differentiation

A

Thymoma - Type B1

Plan:
Correlate with clinical history and radiology (?myasthenia gravis)
Examine further blocks (e.g. for size, relation to margins, areas of differing morphology, TNM / Masaoka-Koga stage)
IPX to support Dx: AE1/AE3, p40 (highlight epithelial cells) , TdT (positive in thymocytes)
Next steps: Synoptic report, discuss at MDT

Q

Encapsulated and multilobulated thymic tumour, with intersecting fibrous bands. Comprised of a mixture of small lymphocytes and polygonal epithelial cells with mildly enlarged nuclei with vesicular chromatin and nucleoli. Occasional medullary islands.

A

Thymoma - Type B2

Plan:
Correlate with clinical history and radiology (?myasthenia gravis)
Examine further blocks (e.g. for size, relation to margins, areas of differing morphology, TNM / Masaoka-Koga stage)
IPX to support Dx: AE1/AE3, p40 (highlight epithelial cells) , TdT (positive in thymocytes)
Next steps: Synoptic report, discuss at MDT

Q

Encapsulated and multilobulated thymic tumour, with intersecting fibrous bands. Comprised of polygonal / epithelioid cells with raisinoid nuclei, with palisading of tumour cells around perivascular spaces. Admixed lymphocytes are few.

A

Thymoma - Type B3

Plan:
Correlate with clinical history and radiology (?myasthenia gravis)
Examine further blocks (e.g. for size, relation to margins, areas of differing morphology, TNM / Masaoka-Koga stage)
IPX to support Dx: AE1/AE3, p40 (highlight epithelial cells) , TdT (positive in thymocytes)
Next steps: Synoptic report, discuss at MDT

Q

Encapsulated and multilobulated thymic tumour, with intersecting fibrous bands. Contains areas of spindled to ovoid bland tumour cells with finely dispersed “powdery” chromatin, admixed with tumour areas with abundant admixed lymphocytes.

A

Thymoma - Type AB

Plan:
Correlate with clinical history and radiology (?myasthenia gravis)
Examine further blocks (e.g. for size, relation to margins, areas of differing morphology, TNM / Masaoka-Koga stage)
IPX to support Dx: AE1/AE3, p40 (highlight epithelial cells) , TdT (positive in thymocytes)
Next steps: Synoptic report, discuss at MDT
