How to calculate predicted post-op respiratory function?
Basically, the calculation uses the number of bronchopulmonary segments removed compared with the total number, 19, in both lungs.
NB: for ppoFEV% predicted, you can just multiple pre-op FEV1% predicted with (19-segments removed/19) see example below
- Example using above:
- For RUL lobectomy = 3 segments removed in a patient with a pre-op FEV1 of 1.6L which is 80% of predicted normal for that patient
- ppo FEV1 = 1.6L x 16/19 —> 1.35L
- ppo FEV1% = 80% x (16/19) —> 0.8 x 16/19 = 0.67 or 67%

COPD
- is a chronic and progressive inflammatory condition affecting central + peripheral airways, lung parenchyma and the pulmonary vasculature
- leads to poorly reversible narrowing of the airways, remodelling of airway smooth muscle, increased numbers of goblet cells & mucus-secreting glands, and pulmonary vasculature changes result in pulmonary HTN
- it is characterised by expiratory airflow limitation because of a combination of small airway inflammation (obstructive bronchiolitis) and parenchymal destruction (emphysema)

Chronic Obstructive Pulmonary Disease (COPD) is a heterogeneous lung condition characterized by chronic respiratory symptoms (dyspnea, cough, sputum production and/or exacerbations) due to abnormalities of the airways (bronchitis, bronchiolitis) and/or alveoli (emphysema) that cause persistent, often progressive, airflow obstruction.
