Hypertension
THIAZIDE DIURETIC - when is it best to take?
Best to take in the morning so diuretic effect occurs throughout the day - rather than taking at night and having disrupted sleep
THIAZIDE DIURETIC - MOA
Inhibit Na+/Cl- cotransporter in the DCT of the nephron = prevents reabsorption of Na+/H20 = diuresis = decreases total blood volume = decreases CO = decreases BP
Long term effects = vasodilatation (not fully understood)

Hyponatraemia
Hypokalaemia
Cardiac arrhythmia
Impotence in men
THIAZIDE DIURETIC - interactions
NSAIDs (other than low-dose aspirin) may lower effectiveness of thiazide
Avoid use with loop diuretics
THIAZIDE DIURETIC - contraindications
Avoid use in patients with hypokalaemia and hyponatraemia
THIAZIDE DIURETIC - patient communication - how would you describe to the patient what this drug does?
Explain to patients that you are giving them a “water tablet” for their high blood pressure (if applicable and with their leg swelling) - it will basically work by making you urinate more which will in turn decrease the amount of fluid circulating in your body and decrease your blood pressure
